This Notice applies to healthcare services provided by Asōka MedSpa to the extent Asōka MedSpa is subject to the Health Insurance Portability and Accountability Act (“HIPAA”).
Asōka MedSpa is committed to protecting the privacy and security of your health information.
Where HIPAA applies, we are required to:
• Maintain the privacy and security of your Protected Health Information (“PHI”);
• Follow the privacy practices described in this Notice;
• Provide you with a copy of this Notice;
• Notify you following certain breaches involving unsecured PHI as required by law;

Treatment:
We may use and disclose your health information to healthcare professionals involved in your care.

Asōka MedSpa does not sell your Protected Health Information.

For example, you may request that we contact you only on a specific telephone number.

We are not always required to agree to a requested restriction.
Where required by law, if you have paid in full out-of-pocket for a healthcare service, you may request that certain information concerning that service not be disclosed to your health plan for payment or healthcare operations purposes
You may request information about certain disclosures we have made of your health information during the period permitted by law.
You may request a paper copy of this Notice at any time even if you previously received it electronically.
A person legally authorized to act on your behalf may exercise applicable privacy rights for you after we verify the person's authority.
